Admissions Steps

Thanks for your interest in Faith Based Christian Academy (FaithBCA). Admissions are currently open for the new school year. The following steps show all you need to know to complete the application process.

STEP 1

1. Click this K-5 Student Enrollment Application and K-5 Student Enrollment Financial Agreement PDF form to view the form.
2. Download the K-5 Student Enrollment Application and K-5 Student Enrollment Financial Agreement form: To download the form click File > Download or click the Download icon.
3. Find the K-5 Student Application Forms Fillable PDF.pdf file in your downloads folder and click to open it with your preferred PDF viewer application.If you do not have a PDF viewer you can download a free version of Adobe Acrobat Reader.
4. Fill in all relevant fields, attach your digital signature and save the file.

STEP 2

Prepare all required documentation; digitized copies for online submission or hard copies for in person (offline) submission.

Required Documents

1. A Completed K-5 Student Enrollment Application and K-5 Student Enrollment Financial Agreement form
2. Copy of ID
3. Copy of Student Health Exam
4. Copy of Student Immunization
5. Copy of Student Birth Certificate
6. Receipt of the Non-Refundable Registration Fee Payment

STEP 3 (For Offline Application)

Bring the completed K-5 Student Enrollment Application and K-5 Student Enrollment Financial Agreement form and required documents to the Reception Desk at the Academy and pay the non-refundable $150.00 registration fee or, if you qualify, the non-refundable $90.00 early registration discount fee. (See the Early Registration Discount qualification requirements below).

STEP 4 (For Online Application)

Early Registration Discount

Qualifying For The Early Registration Discount

To qualify for the Early Registration Discount you must:

1. Submit your payment online or in person within the stipulated early registration period (it will be published at the academy and on the website). If online, you must submit payment via the Early Registration Pay Link or QR Code. The link will be made available during the early registration period.
2. Be in good financial standing with the academy (all fees for prior services paid in full).

If you qualify then you can pay and receive the Early Registration 40% Discount via the following link or QR Code.

1. Enjoy a 40% discount. (Save $60.00).Pay the non-refundable $90.00 Early Registration Fee - payable online via this Pay Early Registration Discount Link, OR scan the following QR Code
   
2. Enter your credit card info and submit your payment.
3. Save a copy of your receipt.
4. Send your completed K-5 Student Enrollment Application and K-5 Student Enrollment Financial Agreement form, required documents and a copy of the $90.00 early registration fee receipt to: admissions@faithbca.com.

Your application submission is complete only when (1) the K-5 Student Enrollment Application and K-5 Student Enrollment Financial Agreement forms are complete and submitted with all required documents, (2) the non-refundable $90.00 early registration fee is paid and (3) the registration fee payment receipt is received.

If you do not qualify, then you can register with the normal Pay Registration Fee Link that follows.

Pay Registration Fee

1. Pay the non-refundable $150.00 registration fee - payable online via this Pay Registration Fee Link, OR scan the following QR Code
   
2. Enter your credit card info and submit your payment.
3. Save a copy of your receipt.
4. Send your completed K-5 Student Enrollment Application and K-5 Student Enrollment Financial Agreement form, required documents and a copy of the $150.00 registration fee receipt to: admissions@faithbca.com.

Your application submission is complete only when (1) the K-5 Student Enrollment Application and K-5 Student Enrollment Financial Agreement forms are complete and submitted with all required documents, (2) the non-refundable $150.00 registration fee is paid and (3) the registration fee payment receipt is received.

STEP 5

Your application and documents will be reviewed and we will inform you of our final decision. The Admissions Office will then provide guidance on the subsequent steps for enrollment.
