Admissions Steps
Thanks for your interest in Faith Based Christian Academy (FaithBCA). Admissions are currently open for the new school year. The following steps show all you need to know to complete the application process.
STEP 1
- Click this K-5 Student Enrollment Application and K-5 Student Financial Enrollment Agreement PDF form to view the form.
- Download the K-5 Student Enrollment Application and K-5 Student Financial Enrollment Agreement form: To download the form click File > Download or click the Download icon.
- Find the K-5 Student - Financial Application Forms Fillable PDF (Digital Submission).pdf file in your downloads folder and click to open it with your preferred PDF viewer application.If you do not have a PDF viewer you can download a free version of Adobe Acrobat Reader.
- Fill in all relevant fields, attach your digital signature and save the file.
STEP 2
Prepare all required documentation; digitized copies for online submission or hard copies for in person (offline) submission.
Required Documents
- A Completed K-5 Student Enrollment Application and K-5 Student Financial Enrollment Agreement
- Copy of ID
- Copy of Health Exam
- Copy of Immunization
- Copy of Birth Certificate
- Receipt of the Non-Refundable Registration Fee Payment
STEP 3 (For Offline Application)
Bring the completed K-5 Student Enrollment Application and K-5 Student Financial Enrollment Agreement form and required documents to the Reception Desk at the Academy and pay the non-refundable $150.00 registration fee.

STEP 4 (For Online Application)
- Pay the non-refundable $150.00 registration fee - payable online via this Pay Registration Fee Link, OR scan the following QR Code
- Enter your credit card info and submit your payment.
- Save a copy of your receipt.
- Send your completed K-5 Student Enrollment Application and K-5 Student Financial Enrollment Agreement form, required documents and a copy of the $150.00 registration fee receipt to: admissions@faithbca.com.
STEP 5
Your application and documents will be reviewed and we will inform you of our final decision. The Admissions Office will then provide guidance on the subsequent steps for enrollment.
